  • Speak up about your wishes and concerns
  • Prepare you for and accompany you to meetings with professionals
  • Help you ask questions about things which are important to you
  • Understand your rights and options

  • Find it difficult to speak up for yourself
  • Need help understanding decisions about your care or support
  • Do not have family or friends who can support you
  • Are involved in health or social care decisions
